Please describe what happened. My 81 year old father was admitted for triple bypass surgery. He contracted pseudomonas from the ventilator tubing. The muscles in his throat were also damaged so that he later had swallowing issues. He then contracted Clostridium Difficile. A few weeks later, still in the hospital, he signed DNR papers. He was coded twice following that. The Dr. stated he did not see the DNR papers. Five weeks following the surgery while still in the hospital he died.
